The CVHCare Patient Care Coordinator (MSW) is responsible for the administrative and clinical coordination of multidisciplinary care teams within a home health setting. Requirements

• Acting as the administrative liaison between the agency, patients, caregivers, payers, and providers.
• Assisting in managing patient care and ensuring quality service.
• Monitoring the work of schedulers and other administrative personnel.
• Maintaining accurate and up-to-date records in the EHR.
• Communicating with patients and families regarding care plans.
• Facilitating interdisciplinary team meetings and coordinating patient care.
• Conducting follow-up visits with field clinicians.
• Performing data entry and record keeping.
• Ensuring compliance with regulations and Medicare/Medicaid requirements.
