Job Description
We're seeking a Case Manager Registered Nurse to join our team at CVS Health. As a case manager, you will work with members, providers, and community organizations to address the full continuum of health care and social determinant needs. You will be responsible for developing, implementing, and supporting health strategies, tactics, policies, and programs to ensure the delivery of benefits and promote member wellness and successful return to work.
Requirements
- Minimum 3 years clinical practical experience
- Minimum 2 years CM, discharge planning, and/or home health care coordination experience
- Registered Nurse with active state license in good standing within the region where job duties are performed
- Certified Case Manager is preferred
