Case Manager

The Case Manager is responsible for coordinating hospital’s Case Management for patients with the goal of providing effective discharge planning to reduce recidivism. Duties include assessing, planning, implementing, coordinating, monitoring, and evaluating options to facilitate the continuum of care, including follow-up appointments, medication/prescription management, housing, and transportation arrangements upon discharge.

Requirements

• Degree in field of Social Services or BSW preferred
• Bi-lingual English/Spanish preferred
• Must obtain and maintain approved CPR certification as well as successful completion of Management of Assaultive Behavior training prior to assuming patient care responsibility
• Valid Texas counselor or nursing license a preferred
• Two (2) years’ experience in a psychiatric health care facility, direct practice in mental health treatment
• Experience in crisis intervention and customer service preferred
• Bi-lingual English/Spanish a plus
