Job Description
Provides case management assessment, person centered planning, and documentation, referral and linkage, and monitoring/follow-up for clients. Works with interdisciplinary teams to develop and implement care management plans.
Requirements
- Provides care management assessment and development of care management plans.
- Refers and links clients to needed services and monitors follow-up.
- Provides education for health promotion and ensures metrics for outcomes are met.
- Participates in interdisciplinary treatment planning and consultation activities.
- Provides crisis intervention consultation to all participants of TCM and involves crisis services when needed.
