Accountable for coordinating care across the continuum for an identified group of patients, participating in the development of plans of care and ensuring quality, clinical, and cost outcomes.
Requirements
- Three or more years acute care nursing experience
- Current NJ - RN license
- Bachelors degree in related field preferred
- Excellent interpersonal skills
- Working familiarity with personal computer
- Familiarity with ICD-9 coding
