System Navigator HART Hub -PFT

Durham Community Health Centre (DCHC) is seeking a System Navigator to support individuals experiencing homelessness and/or struggling with addictions and/or mental health challenges. The role involves engaging with clients, providing case management, and connecting them with relevant resources, fostering relationships with partners, and advocating for their needs within the Harm Reduction model. The position is part of a team-based, trauma-informed, and culturally safe approach, aiming to improve engagement and continuity of care across the region.

Requirements

• Bachelor of Social Work, Nursing, or other relevant health discipline.
• 2 years work experience in the Health and/or Social Work field, or an equivalent blend.
• Registration with a regulated professional body (e.g. OCSWSSW, CNO).
• Experience working with marginalized populations.
• Experience with system navigation, care coordination, or case management.
• Knowledge of local systems (housing, shelters, ODSP/OW, healthcare, harm reduction, and addictions services).
• Strong knowledge of local systems including Ontario Health and Benefits, Housing systems, Employment law
• Valid driver’s license.
