Case Manager - Brooklyn Collaborative Legal Engagement and Addiction Response

EAC Network is seeking a Case Manager to join its Brooklyn CLEAR program, supporting youth and families through assessments, referrals, and case management.

Requirements

• CASAC or Bachelor's in social work required
• Experience working with clients with substance use conditions required
• Knowledge of target population and borough resources
• Excellent organizational and time management skills
• Interpersonal skills, verbal and written communication skills
• Proficient in computer applications and/or database software
