This position will assist in the examination and treatment of patients in the clinic.
Requirements
- Meet with patients in a clinic setting to assist in performing routine tests for the evaluation, diagnosis, and treatment of patients.
- Identify health status by taking health histories, observing signs and systems, taking vital signs, and preparing patients for the doctor to examine.
- Report to the physician any changes in the patient’s physical and emotional condition.
- Participate in the supervision and training of aides and volunteers.
- Keep current and accurate records.
- Interact professionally with the patient and the patient’s family or representatives.
- Consider patient cultural, psychological, social, and environmental factors.
- Respond to medical emergencies as needed.
- Participates as a PCMH Care Team Member and performs the following:
- Prepares for, attends and participates in team meetings and huddles.
- Collaborates in developing team priorities and patient goals and care plans.
- Active in patient education, goal setting, self-management teaching and coaching, and behavior change.
- Medication reconciliation and education.
- Utilizes standing orders when indicated.
- Chronic disease management.
- Perform other duties as assigned.
