Job Description
Elara Caring is seeking a Registered Nurse Case Manager to join their team. The successful candidate will contribute to the company's mission of providing high-quality care to patients in their preferred home setting.
Requirements
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- 50% travel required
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs
- Valid driver’s license and insurance and reliable transportation to perform job tasks
