Job Description
Ennoble Care is seeking a full-time RN Case Manager to work within the Mercer County, NJ area. This role involves assessing, planning, implementing, and evaluating patient needs in accordance with physician orders, providing comprehensive care, and supporting loved ones/caregivers. The RN will work as part of an interdisciplinary team, documenting patient information, and ensuring quality care for patients and their families.
Requirements
- Current RN license in New Jersey
- Two years of professional nursing experience, preferably in hospice
- Valid driver’s license, reliable vehicle
