Job Description
Essen Health Care is seeking an experienced Director of Care Management & Remote Patient Monitoring to lead the growth of their House Calls division. This role will oversee a multidisciplinary team focused on proactive, patient-centered care, advanced primary care management, chronic care management, principal illness navigation, behavioral health integration, and community health integration initiatives.
