Job Description
Essen Health Care is seeking a Patient Navigator to provide coordinated care to members with chronic health conditions. The role involves acting as an intermediary between providers and patients, coordinating services, and navigating social service needs. It’s a Community Health worker role in a fast-paced environment.
Requirements
- Conduct outreach activities specifically to patients with loss to care.
- Conduct community outreach visits to patients.
- Provide expedited visit to patients for urgent situations.
- Assist in busy Medical Office tasks.
- Call, schedule and confirm patient appointments.
- Enrolling patients to Essen Health Care's program.
- Assist in achieving fewer emergency room visits and hospitalizations.
