Care Coordinator, Residential

We are looking for a Care Coordinator, Residential to provide case coordination to clients, families and collaterals, and advocate for client and family in navigating human service and similar department, agencies, and services.

Requirements

• Bachelor’s Degree in social work-related field or significant residential/childcare experience
• Excellent communication skills
• Ability to travel to and from clients’ residence, community locations and office site
• Ability to lift up to 20lbs
• Current driver’s license and reliable transportation
• Knowledge of and sensitivity to cultural, ethnic, racial and socioeconomic issues
