Job Description
The Community Health Worker (CHW) role involves providing patient support and addressing Social Determinants of Health (SDoH) within Fenway Health clinical locations. The CHW will collaborate with medical providers and other agencies to improve patient care and outcomes. Duties include providing care coordination, assisting patients with enrollment in programs, documenting patient interactions, and conducting outreach to community resources. The role requires sensitivity and cultural understanding.
Requirements
- High School Diploma
- Knowledge of MS Office suite, particularly Excel and Outlook
- Strong organizational and attention to detail skills
- Ability to handle multiple tasks and prioritize assigned duties
- Ability to work harmoniously and effectively with diverse groups
- Willingness to contribute to anti-racist efforts
