Enhanced Care Management (ECM) Program Manager

The Enhanced Care Management (ECM) Program Manager oversees the implementation of the ECM program for young children and families with complex medical and social needs. The role ensures program quality, partner alignment, and regulatory compliance, and supervises staff to deliver high-quality, coordinated services.

Requirements

• 5+ years of experience in care management, case management, or health/social service delivery
• 3+ years of program management and supervisory experience
• Master’s degree in social work or a related health field
• Current and active License in one of the following: LCSW, LMFT, LPCC
