Obesity Medicine Nurse Practitioner

Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. We are hiring experienced Nurse Practitioners to provide longitudinal telehealth care for patients with obesity.

Requirements

• Nurse Practitioner degree with experience practicing Obesity Medicine
• Licensed to practice and prescribe legally in FL, with preference for additional licenses in MA, IL, NY, and/or TX
• 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care/internal medicine setting
• Excellent interpersonal skills for patient and team interactions
• Committed to the highest quality of medical care and an evidence-based approach to obesity management
• Prior experience conducting telemedicine visits
• Bilingual candidates with medical fluency in Spanish are highly preferred
