Case Manager

The Case Manager will assist tenants in making the transition from criminal justice history and homelessness to positive, legitimate and productive lives while residing in permanent supportive housing. The Case Manager is responsible for the day-to-day activities of their assigned caseload, including assessing, counseling, and connecting tenants to additional services.

Requirements

• High school diploma or GED required, college preferred (AA or BA level)
• At least 2 years' experience working with justice impacted seniors ages 62+, chronic medical conditions, behavioral health (substance use and/or mental health) populations
• Experience using Microsoft Office programs and case management database systems i.e. Case Worthy
