Program Manager

The Fortune Society is seeking an experienced NYS Health Home Care Management supervisor to manage the operations of its care management department. This onsite position will have overall day-to-day responsibility for ensuring the connection of criminal legal system impacted adults to medical, mental, and social determinants of health services.

Requirements

• Minimum of a master's degree in a qualifying field or CASAC
• At least 3 years of progressive experience in NYS Health Home Care Management
• Experience with Health Home Plus (inclusive of HARP), Serious Mental Illness (SMI) high needs individuals, including best practices and state/health home regulations
• Minimum of 2 years' experience in a supervisory capacity
• Ability to demonstrate strong written and verbal communication skills through reports, correspondence and presentations
• Strong organizational and interpersonal skills; ability to manage competing priorities within a growing unit
• Proficiency in MS Office, SharePoint, and Excel, as well as MAPP, ePaces, PSYCHES
