Health Coach Specialist II

FreedomCare is a rapidly growing, consumer-directed home care agency revolutionizing the delivery of care in home and other settings. This Health Coach Specialist II role is field-based, requiring a candidate to act as a central point of contact for customers and provide support to waiver members, caregivers, and other stakeholders in Connecticut. This position emphasizes customer service, ensuring patients receive the appropriate level of care, and fostering independence in their home environment.

Requirements

• Bachelor's degree or equivalent work experience
• 2+ years of experience working with frail or elderly disabled adults and caregivers
• 2-3 years Inside Sales, call center, telephonic outreach, customer service or other similar experience
• 1-3 years of medical field experience and/or case management, home health
• Ability to exercise judgment independently
• Strong communication and interpersonal skills
• Ability to manage time and prioritize tasks
• High level of empathy and emotional intelligence
