Job Description
FutureCare is seeking a Licensed Director of Social Work to advocate for residents and improve access to health care services within their facilities. The role involves providing crisis intervention, promoting resident well-being, and providing Clinical Supervision. Requirements
- Coordinate and supervise medically related social services.
- Supervise programs designated to the Social Work Department.
- Participate in daily meetings with the interdisciplinary team.
- Provide individual, family, and group services.
- Address advance directives and assist in determining medical decision-makers.
- Oversee and coordinate discharge planning.
- Prior experience in a skilled nursing and Long-Term Care environment
- Management experience in an interdisciplinary setting
- Excellent interpersonal skills
- Ability to navigate electronic health record systems
