Director of Home Health (PACE Modesto)

The Director of Home Health Services oversees all aspects of the Home Health program and associated patient care and staff providing care in this program. Responsibilities include regulatory and compliance oversight, patient and staffing assignments, coordination of care, assessment of patient needs and delivery of compliant care in participant’s home, skilled nursing facility, assisted living or group home and hospital discharge/continuity of care environments.

Requirements

• Develops, plans and implements all aspects of the Home Health Program
• Responsible for the delivery of care for all patients receiving care through the Program
• Coordinate with Interdisciplinary Teams (IDT) to insure collaboration and coordination of services and care plans
