Hospice LPN
Requires LPN license, previous experience in long term care, home health, acute care nursing, or hospice. Must be able to work in collaboration with others, good communication and human relation skills
Hospice Massage Therapist
Minimum 1 year of clinical massage therapy experience and currently licensed as a massage therapist in the state. Training in geriatric massage preferred
Hospice Weekend On Call Visit Social Worker
Graduate of accredited school of Social Work, Current state of Minnesota Social Work license, Three years' recent experience in acute, long-term care or home care/hospice/public health
Hospice RN Case Manager
RN licensure, 3 years experience, patient care planning and teaching skills, written and verbal communication skills, critical thinking
Hospice Patient Care Manager
3+ years of hospice, long term care or community health experience; current Minnesota RN licensure; associate or bachelor’s degree
Hospice Chaplain
Master's in Theology, 4 CPE credits, and Ecclesiastical endorsement are key. Experience in death/dying and hospice is also essential
Hospice Social Worker
MN social work license is required. Master's or Bachelor's in Social Work with 2+ yrs experience preferred. Clinical social work experience with individuals/families needed
Hospice Home Health Aide/CNA
CNAs with 1+ year of experience; valid MN driver's license. Must use tablet/mobile to learn electronic charting
Hospice Weekend Visit Nurse
Registered nurse or LPN in MN with 3+ years experience. Requires proficiency in acute, long-term care or hospice care. Must be patient focused with excellent communication skills
Hospice Resource RN
3+ years of experience in nursing, ability to use MS Outlook and EHR, and current Minnesota RN licensure required
