Hospice Social Worker

Grace Hospice provides compassionate end-of-life care to terminally ill patients and their families, aiming to improve quality of life and dignity. The hospice social worker provides services in accordance with Medicare guidelines and under the direction of a physician, offering consultations, assessments, care planning, and counseling. The role involves a team-based approach and a strong focus on the patient and family's needs.

Requirements

• Social Work Licensure in the state of Minnesota
• Masters in Social Work preferred, from an accredited program; BSW with at least two years of experience
• Clinical social work experience with individuals and families
• Knowledge and experience with family systems theory, individual and family counseling, and group process
• Knowledge of community resources including Medicare, Medicaid, the hospice benefit
• Critical thinker with strong problem-solving skills
• Ability to work independently, take initiative and think creatively
• Experience managing interpersonal dynamics
• Current Minnesota driver’s license, insurance and reliable daily transportation
