Job Description
Ontario Health atHome is seeking an experienced Care Coordinator to assess patient needs, provide referrals to community services, and engage with healthcare practitioners. Requirements
- Membership in the applicable regulatory body
- 1+ years of recent experience in community health or related field
- Knowledge of the health care delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving and decision-making skills
- Effective time management, prioritization and organizational skills
- Established ability to complete required documentation
- Valid driver’s license and access to a reliable vehicle
- Proficient in a Windows environment
