Field RN Case Manager

Field Registered Nurse Case Manager will work with Medicare Members with complex medical or behavioral health conditions or who are at high risk. The role will involve traveling 60-70% of the time and working remotely for the remaining time. The candidate will assess and create care plans for members and coordinate services between healthcare providers.

Requirements

• 5+ years of clinical nursing experience
• 3 years of clinical experience with case management experience
• 2+ years of related experience in managed care environment (Medicare experience) preferred
