Job Description
The Lead Street Outreach Case Manager is responsible for leading outreach teams and providing direct services to individuals experiencing homelessness, while also supporting team members and collaborating with community partners to connect clients to shelter and housing solutions.
Requirements
- High school diploma required
- 4 years of experience with people experiencing homelessness and people with mental/physical health problems
- Available to work assigned schedule including evenings and weekends
- Working knowledge of HMIS and other Homeless outreach databases
- Strong time management skills and ability to multi-task and prioritize
- Excellent communicator with ability to maintain professionalism in crisis situations
- Reliable transportation and valid driver's license
- Ability to pass TB and background clearance
