The Case Coordinator, CalAIM, supports the development, implementation, and expansion of CalAIM Program-Community Supports, ensuring programs and resources align with HumanGood’s goals and CalAIM regulations across several affordable housing communities.
Requirements
- Bilingual: Ability to communicate in Spanish and/or Chinese is preferred
- Bachelor’s Degree in Gerontology, Social Work, Public Health, Healthcare Administration, or related field
- 2+ years of experience serving older adults
- 1+ year of experience in case management
- Experience working with vulnerable individuals is preferred
- Experience with administration of various assessment tools and creating, implementing and monitoring care plans for residents and care coordination with other providers
- Working knowledge of social, economic, and political issues relevant to low-income communities
- Familiarity with trends in the field of healthcare, housing and social services
- Understanding of current federal, state and local laws, regulatory codes, ordinances, procedures relevant to social services programs
- Principles and practices of performing needs assessments; program development, implementation, management and evaluation
- Conflict resolution and problem-solving techniques
- Experience with community outreach, advocacy and public education
- Principles and practices of staff management, supervision and training
- Strong oral, written, presentation, interpersonal and diplomacy skills
