Job Description
The Care Manager II provides support to youth and adults in their community setting with the goal of reducing emergency room visits and decreasing Medicaid costs. This role involves comprehensive assessments, care planning, and utilization of community resources to coordinate services for adults, youth, and families. The Care Manager is a vital member of the team responsible for the client and acts as a liaison between the program and external resources.
