Community Support Facilitator

Hope Services is seeking a Community Support Facilitator to provide support and leadership for consumers to participate in activities and receive services in community settings. The role involves developing opportunities for consumers to participate as valued and contributing members of the community, facilitating and modeling interactive relationships with community members, and providing group and/or individual support in the community.

Requirements

• High school diploma, GED, or equivalent
• Two years of related experience or approved combination of experience and education
• Valid California driver's license and clear driving record
• CPR and First Aid certification
• Ability to work independently and cooperatively as a member of the team
• Excellent communication and documentation skills
• Knowledge and understanding of alternative communication systems and assistive technology
