Job Description
Under the supervision of the Case Management or the Community Based Services Supervisor, the Case Manager provides high quality case management to Human Options clients, including comprehensive assessments, care planning, and coordination of care.
Requirements
- Bachelor’s Degree in social work, psychology, counseling, sociology, behavioral science, or related field
- Three years of experience providing direct services to the target population
- Two years of experience in case management with at risk children and families
- One year's knowledge of the child welfare system
- Valid CA Class C license and auto insurance or ability to travel to meetings, trainings, and home visits
- Bilingual in English and Spanish
- Proficiency in Microsoft Office and working knowledge of computer database program entry and report generation
