Job Description
We are seeking a Temporary Full-Time Social Worker to join our Allied Health team at Humber River Health. As a Social Worker, you will provide psychosocial assessment, crisis intervention, discharge planning and counselling to patients and their families. You will also act as a liaison and assist patients and staff with community resources.
Requirements
- MSW obtained from an accredited school of Social Work
- Current membership with OASW and OCCSWSSW, or eligibility to become a member
- Experience in the acute and/or sub-acute sector system, patient flow and discharge planning is an asset
- Computer skills
- Ability to manage electronic recording, workload and meet submission timelines
