Transition of Care Management Care Navigator

The Transition of Care Management Nurse will promote effective partnerships among patients, families, and healthcare providers to coordinate care for patients transitioning back to their home after a hospital or long-term care facility stay. The TOC nurse will provide clinical health follow-up and coordinate care to help patients transition back to a community setting.

Requirements

• 3+ years of experience in a clinical setting providing Care coordination, case management, home health or behavioral healthcare
• Current state licensure as a Licensed Practical Nurse or Registered Nurse
• Outstanding verbal communication and documentation skills
• Strong clinical assessment and critical thinking skills
• Ability to multitask and deal with rapidly changing priorities
