Job Description
Illumination Health + Home is seeking a Lead Care Manager to provide comprehensive care and coordination services to clients temporarily housed with the organization. The role involves client engagement, enrollment, monitoring, and collaborating with team members to ensure service delivery and support program outcomes. Strong knowledge of social determinants of health and care coordination is required.
Requirements
- Bachelor’s degree in social services, Healthcare, or related field; or equivalent combination of training and experience.
- Current valid CA driver's license.
- Experience in homeless services, care management, and mental health support.
- Currently at risk/unhoused individuals.
- Familiarity with VI-SPDAT (if applicable).
