Job Description
Imagine the Possibilities is seeking a Community Integration Professional I to support individuals in achieving community inclusion, skill development, and participation in recreation and volunteer opportunities. This role focuses on identifying individual needs, planning activities, and providing support through behavioral care plans to help people thrive. The position is integral to a Day Habilitation team and involves collaboration with various departments.
