Physician-PACE

Jordan Valley Community Health Center (JVCHC) is seeking a highly skilled PACE Physician to provide primary care services to elderly patients in a Program of All-Inclusive Care for the Elderly (PACE) setting. The role involves providing comprehensive assessments, developing care plans, and coordinating with interdisciplinary teams to deliver high-quality patient care.

Requirements

• Medical degree from an accredited institution
• Board certification in Internal Medicine or Family Medicine
• Active medical license in Missouri
• Experience working with elderly patients preferred
• Excellent communication and interpersonal skills
• Ability to work effectively in a team-based care model
• Commitment to providing high-quality, patient-centered care
