Adult Primary Care Physician

PACE is seeking a certified Adult Primary Care Physician to provide comprehensive medical and behavioral care to seniors in a Medicare and Medi-Cal population. The role involves providing primary care, coordinating care plans, collaborating with the IDT, and utilizing the innovative PACE program. Emphasis is placed on engaging with patients and offering comprehensive, holistic care.

Requirements

• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
• Master’s degree/doctorate degree
• Board Certification – Internal Medicine or Family Practice
• Board Certification – Geriatrics preferred
• Active California Physician License without restrictions
• State CDS Certification and DEA Registration
• Valid state driver’s license with a clean DMV record
• Current knowledge of chronic care/geriatric issues
• Excellent clinical, organizational, and communication skills
