LifeCenter Northwest is looking for a Clinical Communications Coordinator to provide essential management of hospital referrals for potential donors, including initial response, determination of referral escalation or de-escalation, and ongoing updates. The role requires a dynamic and enthusiastic team member who can work remotely and review hospital medical records, transcribe documentation, and communicate with hospital care teams and LifeCenter colleagues.
Requirements
- Manage referrals for seven 12-hour shifts within a two-week pay period
- Answer death notification referral calls
- Perform initial evaluation of all referral calls
- Interact with hospital staff members to obtain medical information
- Provide exemplary customer service
- Route donor referral calls and/or record messages
- Discuss screening information with LifeCenter clinical coordinators
- Enter data of potential donor referral information into LifeCenter's electronic medical record
- Coordinate and perform ongoing evaluation of referred potential organ donors
- Assess information related to family dynamics
- Perform follow-up calls to support referral stewardship and management
- Accurately complete donor records and/or chart corrections
- Attend continuing education conferences/workshops
- Collaborate with donor hospital staff, recovery partners, and LifeCenter teams
