The Case Manager II provides services for patients with complex care needs, conducting patient outreach, engagement, and psychosocial service assessment, and assisting in developing a patient-centered care plan. This position requires strong organizational, administrative, and problem-solving skills, as well as excellent interpersonal and verbal skills.
