The Case Manager II plays a vital role within the primary care interdisciplinary team, providing services to patients with complex care needs. This position involves patient outreach, engagement, psychosocial assessment, care plan development, Enhanced Case Management (ECM) implementation, and coordination of service referrals. Services are provided in various settings, including homes, clinics, and community locations, supporting patients with multiple health and social needs. The position is full-time and represented by SEIU-UHW, offering a dynamic work environment within a Federally Qualified Health Center.
