Job Description
Link Home Therapy is seeking a Licensed Clinical Social Worker to provide clinical assessments, therapy, and counseling services to geriatric individuals experiencing mental health challenges in two Assisted Living Facilities in Berlin, NJ and Forked River, NJ. The role involves developing individualized treatment plans, providing therapeutic interventions, and collaborating with a multidisciplinary team to support resident well-being.
Requirements
- Licensed Clinical Social Worker (LCSW) required in the state of New Jersey
- Conduct comprehensive behavioral health assessments
- Develop individualized treatment plans
- Provide evidence-based therapeutic interventions
- Coordinate with healthcare professionals
- Provide crisis intervention services
- Maintain accurate clinical documentation
