Job Description
The Case Manager plays a vital role in a Magnet Recognized tertiary academic medical center, serving as a core member of the patient care delivery team. They manage case assignments, conduct assessments, implement care plans, and collaborate with interdisciplinary teams to provide excellent patient-centered care within a fast-paced environment. They are expected to adhere to advanced nursing practices, maintain quality standards, and embody professional roles within the healthcare system.
Requirements
- Bachelor of Science in nursing (BSN)
- Minimum two years of hospital nursing experience
- Minimum two years of case management, utilization review, or discharge planning experience in acute care
- Active and unrestricted California Registered Nurse (RN) license
- Current Basic Life Support (BLS) certification
- Nationally recognized Utilization Management or Case Management certification (preferred)
