The ideal candidate will have at least three to five years of experience in primary care, including administration and program management. The role involves ensuring patients receive optimal care, coordinating care across multiple health settings, and developing care plans to prevent disease exacerbation and improve outcomes.

Requirements

• Current MA RN license
• 3-5 years of experience in primary care
• Comprehensive nursing assessment and problem identification skills
• Care plan development and disease management skills
• Computer skills including word, excel and PowerPoint
