Requirements

• Active Registered Nurse (RN) license in the State of Illinois
• Minimum 6+ years of nursing experience, preferably in primary care, public health, or community health settings
• Experience working with federally qualified health centers (FQHCs) or similar safety-net providers
• Demonstrated knowledge of hypertension, heart disease, and stroke prevention and management
• Strong understanding of clinical quality improvement and performance measurement principles
• Excellent communication, facilitation, and training skills
• Ability to work collaboratively across multidisciplinary teams and with diverse community partners
