Job Description
The Medication History Nurse is responsible for collecting accurate home medication lists for patients in the ED, procedures, or inpatient units. This role requires collaboration with pharmacy and healthcare providers and incorporates essential duties of a Registered Pharmacy Technician (RPhT). The position is full-time, scheduled 7:00 AM - 7:00 PM.
Requirements
- Identify patients admitted to ED or procedures.
- Interview patients regarding home medications.
- Contact pharmacy and healthcare providers for information.
- Verify medication information in the EHR.
- Perform pharmacy technician duties as needed.
- Ability to perform essential duties of a RPhT.
- Proficiency in computer-based operations.
- Strong comprehension of hospital/departmental policies.
