Registered Nurse Case Manager (RNCM)- Hospice

We are seeking a Registered Nurse Case Manager (RNCM) to provide skilled nursing care and support to hospice patients in accordance with state guidelines and hospice policies.

Requirements

• Graduate of an accredited School of Nursing
• Current RN licensure in the state
• Current CPR certification
• Minimum one (1) year recent nursing experience
• Hospice or palliative care experience preferred
• Experience with case management and/or hospice admissions strongly preferred
• Strong clinical, assessment, organizational, and communication skills
• Ability to work independently and collaboratively in home and facility settings
• Capable of meeting the physical demands of patient care
• Valid driver’s license, reliable insured vehicle, and meet organization driving requirements
