Job Description
Molina Healthcare Services (HCS) is seeking a Care Manager (RN) to assess, facilitate, plan, and coordinate integrated care for members with high need potential, including behavioral health and long-term care. This role focuses on ensuring patient progress toward desired outcomes and cost-effectiveness based on illness severity and service setting. The RN will conduct face-to-face visits, monitor care plans, and promote integration of services.
