Job Description
Monogram Health is seeking a skilled Nurse Practitioner to join their team in delivering personalized and compassionate medical care to patients with chronic kidney disease and/or end-stage renal disease/end-stage kidney disease. The Nurse Practitioner will work with an integrated care team to assess patients, provide education, and coordinate care with primary care providers and specialists.
Requirements
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
- Current and unrestricted DEA certificate
- 2 or more years of direct patient care required
- Managed Care/IPA/Health Plan experience
