Job Description
This position works with a designated clinical team to provide case management and support to patients in clinic and community settings. The CHW is responsible for addressing and removing barriers to improve patient care and health outcomes.

- Obtaining or maintaining Traditional Health Worker certification is required
- Excellent communication and interpersonal skills
- Strong, proven computer proficiency
- Knowledge of community social service agencies and programs
- Fluency in Spanish preferred
