Job Description
The Mount Sinai Hospital is seeking a Social Worker - Adolescent Primary Care to provide services to at-risk adolescents and families in an integrated medical and mental health setting. This role involves providing short-term therapy, case management, and crisis intervention, as well as providing psychosocial support and advocating for the needs of youth and their families. The social worker will consult with medical providers and collaborate with other professionals to provide comprehensive patient care.
