Job Description
The Nurse Care Coordinator (RN) plays a vital role in advancing high-quality, patient-centered care within our Federally Qualified Health Center (FQHC).
Requirements
- Current Vermont RN license in good standing
- Associate or Bachelor’s degree in Nursing (BSN preferred)
- Minimum of 2 years of RN experience preferred
- Experience in primary care, care coordination, case management, population health, or community health strongly preferred
- Knowledge of social determinants of health and community-based resources
- Strong communication, organization, and patient advocacy skills
- Commitment to the mission and values of community health and serving underserved populations
